Foundational workshop for Care Coordinators and Service Facilitators with an emphasis on wraparound facilitation skills.
With the principles of wraparound as a foundation, participants will learn how to develop a team and guide them through the wraparound process including completion of a strength-based assessment, determination of underlying needs, development of an individualized plan of care including planning for crisis, and facilitation of transition out of the formal team process.

Each module is around 20 minutes in length, for a total of approximately 2.3 hours of pre-training study.
This workshop is intended for care coordinators and service facilitators who wish to learn the fundamentals of the wraparound process used in CST and CCS with an emphasis on care coordination and team facilitation responsibilities and skills.
Individuals not in the role of care coordinator or service facilitator are welcome to attend, however, the Wraparound Fundamentals for Service Providers, Family and Community Supports may be a better fit.
This workshop is interactive. Active participation for the duration of the workshop is expected.

This event offers Attendance Certificate Hours which may count toward ongoing training requirements for programs such as Comprehensive Community Services (CCS).
This event has not been pre-approved to count toward specific licensure CEU requirements, however, hours may count toward 15 of the 30 required continuing education credit hours for LCSW, LPC, and MFT licensure that may be met through "in-house training and development programs". Please check with your particular licensing requirements for more information.
